A furuncle is a purulent inflammation of a hair follicle and of the tissues surrounding it. On the face this is more dangerous than on the body: the blood from the area of the nose, the upper lip and the cheeks drains away in the direction of the cavity of the skull. That is exactly why such inflammations are not opened in just any consulting room and are not squeezed out at home.
Squeezing it out does the opposite of what is needed: part of the pus is driven deeper into the tissues, and the inflammation spreads further. An incision means cutting the lesion open, removing the purulent contents completely and treating the cavity that is left. The surgeon plans the incision so that the mark left after healing is as inconspicuous as possible.

In the majority of cases purulent inflammation of the face is resolved by an incision and by proper wound care. But there are signs that mean you cannot wait for an appointment:
- a temperature above 38 °C with chills;
- swelling spreading to the eyelid or the eye socket, or spreading rapidly across the face;
- severe headache, nausea, confusion;
- a sharp deterioration in general condition.
In these cases you need to go to an emergency department immediately.

The surgeon assesses the stage of the inflammation and decides whether to open the lesion now or to treat it conservatively first. At an early stage treatment without an incision is sometimes enough - in that case the doctor will say so. Once the purulent lesion has formed, ointment will not dissolve it: the contents have to be let out.
The incision is performed under local anaesthesia in an operating room: the cut itself, complete removal of the purulent contents, irrigation and antiseptic treatment of the cavity. After that the wound is treated until it is completely clean - how many dressings will be needed depends on the extent of the inflammation.

The doctor monitors the wound until it closes and explains how to care for it at home. A mark remains at the site of the incision; with time it usually becomes barely noticeable, but healing without any trace at all cannot be promised after a purulent inflammation.
If purulent inflammations keep coming back, that is a reason to be examined: they often have to do with the general state of the body rather than with the skin. An incision deals with the current focus but not with its cause, and the doctor will say so plainly.
The result is always individual and depends on the diagnosis, the preparation, the scope of the treatment and the recovery.
